First Choice Community Healthcare (FCCH) welcomes all patients, regardless of insurance status, ability, or the inability to pay.

FCCH is a Federally Qualified Health Center (FQHC) supported by federal grant dollars, donations and payments received from patients. Being an FQHC means we are able to offer dental, medical, and behavioral health services through a Sliding Fee Discount Program.


What is a Sliding Fee Discount Program?

A Sliding Fee Discount Program (SFDP) was created to offer healthcare to individuals who are uninsured, under insured, or have low income. The SFDP adjusts the amount a qualified patient owes for their healthcare services based on household income and size. Patients in households with lower incomes will pay less for the SAME healthcare services that are offered at undiscounted cost. You can apply for the SFDP at any FCCH location. Once you are approved for the SFDP your discount is good for 1 year. To continue to receive the discount, you must reapply each year.

How do I qualify?

To qualify for the Sliding Fee Discount Program, you must show proof of gross income for ALL household members. Gross income is all income before taxes. Acceptable proof of income is any of the following:

  • Previous year’s federal tax return showing Gross Income
  • At least 2-4 paycheck stubs
  • Unemployment compensation verification
  • Disability benefits verification
  • Pension or retirement income
  • Educational assistance
  • Alimony
  • Child support
  • Worker’s compensation
  • Supplemental income
  • Veteran’s or Survivor’s benefits
  • Interest
  • Dividends
  • Rental income
  • Royalties and estate funds
  • A signed letter from employer on company letter head confirming gross wages
  • A notarized letter of support from a third party confirming how they are supporting you.
  • All other income

While the SFDP can greatly reduce the amount you owe, it does not mean that the services you receive are free. Payment is due at the time of service. If you cannot pay at the time of service, we will help you make a Payment Plan to pay out the balance of what you owe over time.

Patients who have a balance after 30 days can request a Payment Plan that will help avoid the collections process and to be able to keep getting care at FCCH.

Please remember that FCCH depends on you to help us to keep providing these much-needed services to our communities and patients! You can make payments by mail or in person at any of our locations. Medical patients can use the “pay my bill” option on our website.
