We provide primary care and prevention services in eight health centers and one school-based health center in Central New Mexico. We are accredited by The Joint Commission, which assures the highest quality of care and safety.

Extended and Weekend Hours of Operation

We offer extended hours appointments for established patients at most health centers and Saturday appointments at our Edgewood health center. Check with the health center for earliest or latest available appointment times.

Making appointments

At First Choice, the full range of Primary Care is our most important service. We ask patients to establish a relationship with a provider by scheduling an appointment. This will allow each patient to develop a steady relationship with a provider to discuss and handle both their general health care problems and their preventative health needs.

When you need to be seen on the same day you call

Part of a strong primary care relationship is to be available to our patients when they are ill or in need of more immediate attention. Each of our clinics has same day appointments that we reserve for our established patients that are ill or need attention. One advantage of taking the time to schedule a new patient visit to establish your relationship with a site and a provider is that we prioritize these same day slots for our established patients.

We always encourage patients who are ill and need same day attention to call their clinic first. There are sometimes appointments for later the same day that our front desk staff can offer. If not, they will connect you with one of our nurses to begin to assess your needs and work to identify a good plan with you.

Need A Copy of Your Health Records?

To request a copy of your health records or to give permission for someone else to receive your records, print and fill out this form. Bring it in to the health center where you received services, and we will take care of your request. There is a cost for copies of health records, depending on number of pages.

Paying for Your Care

We ask for payment at the time of service and help people to apply for assistance to help cover the cost of their care. We accept most insurance and offer a sliding scale fee for patients who need help paying their health care bills. We also help people apply for Medicaid, marketplace insurance and special programs such as the Breast and Cervical Cancer Program.
