Fee Discount Based on Income and Family Size

We make every effort to help our patients get help paying for their health care. We screen all patients for eligibility for Medicaid and assistance programs. We offer a self-pay discount based on family size and income. Patients with private insurance can also apply for help to cover high deductibles.

To qualify for the sliding fee scale for a full year, please bring:

• Proof of income such as a tax return with W-2 forms and 1099s from the prior tax year; check stubs for the prior month, or a notarized letter from employer or source of support confirming the gross pay of patient or how they are being supported. We have blank letters that you may use.

To qualify for the sliding fee scale for a one-time discount:

A patient may self-disclose their income on an income affidavit form to receive a discount for one visit. To receive an additional discount, patients must bring in proof of income such as tax return from the prior tax year, check stubs for the prior month or a notarized letter from a third party (such as employer, landlord or someone with whom they are not living) stating the patient's income or source of support for living expenses.

Open Enrollment for the insurance marketplace begins Nov. 15, 2014 for coverage that begins Jan. 1, 2015. You can apply for health insurance coverage online by going to Healthcare.gov or you can apply by phone by calling the Federal Call Center: 1-800-318-2596 or in person at First Choice.
