Behavioral Health

Behavioral Health (counseling) services are available to patients who are established Medical or Dental patients of First Choice. Services are provided at all of our sites and we offer bilingual services at our South Broadway, South Valley, Los Lunas, Alameda and Alamosa sites. To be referred for Behavioral Health services, you must be referred by your Medical or Dental provider.

 Within our Behavioral Health Program we offer the following services when referred by your provider:

  • Individual Counseling for adults
  • Individual Counseling for children/adolescents ( age 4 to 17 ) at our South Valley site
  • IOP ( Intensive Outpatient Program ) for adults who struggle with substance use issues at our South Valley site
  • MAT ( Medically Assisted Treatment ) for adults who struggle with Opioid use/dependence
  • Community Health Workers who can assist with resources

Our Behaviorists utilize Evidence Based Practices to provide clinically competent services to our patients. We address concerns around:

  • Depression
  • Anxiety
  • Substance use
  • Life stressors
  • Psychiatric services ( for established patients only with referral from provider) 

For an appointment, please contact the health center where you are established. Our providers may also make referrals to other behavioral health resources available in the community.

  • We assist with Medicaid applications
  • We help patients find insurance on the Marketplace
  • We enroll patients in special programs when they are eligible
  • We provide a discount for services based on family size and income
