Minimum Requirements:

High school graduate or equivalent; Previous successful experience in a community service program or outreach preferred; Bi-lingual in English and Spanish preferred Under direct supervision, and in accordance with care plans, performs outreach and liaison services to patients and families of FCCH participating in the ECHO-Cares program for the management and promotion of health and wellness for patients with complex health care needs.  Work may be performed in one or more of focus areas and settings: Community Health Workers (CHW’s) will be focusing on specific components of care: Make referrals and perform interventions related to the social needs of the patient Assist patients with access to resources and social services Facilitate client access to information, including formal and informal support systems Provide social motivational support for behavior change Develop and manage support groups as needed

Provide health coaching and facilitate goal setting Conduct home visits as part of the care plan Make presentations on behalf of the project to local communities Make the patient feel connected with FCCH Will need to conduct some outreach through home visiting, at different clinic sites, or facilitate discharge requirements for hospital admissions  Will need to support data collection and data entry into ECHO program and FCCH electronic health records.
