First Choice Community Healthcare makes every effort to provide the highest quality, timely care to our patients. We try to see patients who are sick within two days. Triage may be necessary on those days when circumstances or staffing do not permit a patient to be seen the same or next day.

What to Bring

For every visit, patients should bring a list of all current medications and proof of health insurance or other financial assistance. If patient has no insurance or assistance, please bring evidence of income (last year’s tax return, last month’s pay stubs or letter to show patient’s income). You may qualify for a discounted fee. How much will it cost?

Co-payments are due at the time of service. Patients without health care coverage can apply for our sliding fee discount based on income and family size. Self-pay patients who qualify for the discount should expect to pay at least $30, the minimum co-pay for a medical visit. There may be additional charges, based on the care provided during the visit and/or the level of eligibility for sliding fee adjustment.

First Choice may be able to identify additional funding support for our patients. Certain programs may help patients meet their financial responsibilities for the health care they receive. Click here for eligibility.
